Bedtime rumination mediates the association between burnout and insomnia in emergency department staff

This study found that cognitive pre-sleep arousal, specifically bedtime rumination, fully mediated the relationship between burnout and insomnia in emergency department staff. Vulnerable shift adaptors, identified by lower-than-expected sleep metrics given their schedule load, reported higher burnout, emotional exhaustion, and insomnia. The findings support a cognitive-behavioral framework linking stress-related cognition to both subjective sleep complaints and objective shift adaptation.

Frequently Asked Questions

What is bedtime rumination?โ–ผ

Bedtime rumination refers to cognitive pre-sleep arousal, such as worrying or thinking about work-related stressors while trying to fall asleep.

How was shift adaptation measured?โ–ผ

Shift adaptation was classified using actigraphy-derived total sleep time and sleep regularity, comparing actual sleep metrics to expected values based on schedule-related load.

Did the study test any intervention?โ–ผ

No, this was a secondary pre-intervention analysis of the REST randomized controlled trial, meaning no intervention was tested in this analysis.

What population was studied?โ–ผ

The study included 58 physicians and nurses working in emergency departments, a group with high prevalence of burnout and sleep disturbances.
