Brief bedside PPG-HRV, particularly HF-related indices such as lnHF and HFnorm, provides auxiliary physiological information related to pain, sleep disturbance, and depressive symptoms in hospitalized adults with chronic pain.
In a cross-sectional study of 172 inpatients with chronic pain, lnHF was significantly associated with pain, sleep disturbance, and depression. Random-forest models showed the strongest predictive performance for sleep disturbance (R²_CV = 0.844), followed by depression (R²_CV = 0.696) and pain-related burden (R²_CV = 0.506).

Limitations
- ⚠Cross-sectional design precludes causal inference
- ⚠HRV is not suitable for diagnosis or stand-alone decision-making
Frequently Asked Questions
What is PPG-HRV?▼
PPG-HRV stands for photoplethysmography-derived heart-rate variability, a non-invasive method to measure beat-to-beat changes in heart rate using a fingertip sensor.
How well does HRV predict sleep disturbance in chronic pain patients?▼
The random-forest model showed strong predictive performance for sleep disturbance with a cross-validated R² of 0.844.
Does sleep affect the relationship between pain and emotions?▼
Yes, sleep disturbance significantly moderated the pain-emotion pathway, amplifying the association between pain and both anxiety and depression.
Can HRV be used to diagnose chronic pain?▼
No, HRV is not suitable for diagnosis or stand-alone decision-making but may serve as a practical adjunctive marker for risk stratification and symptom monitoring.
