Early glucose peak time during OGTT is a stronger biomarker of metabolic health than glucose curve morphology in normoglycemic adults

In normoglycemic adults, an early glucose peak (≤30 min) during a 3-hour OGTT was associated with lower post-prandial glucose, lower 3-hour insulin AUC, lower 2-hour FFA AUC, lower respiratory quotient, lower body fat, and higher energy expenditure compared to a late peak (>30 min). Biphasic glucose curves showed lower post-prandial glucose but no differences in insulin sensitivity or other metabolic measures versus monophasic curves. Glucose peak time demonstrated a stronger discriminative index of metabolic health than curve morphology.

Study Evidence

Study 1. Glucose Curve Morphology and Peak Time as Biomarkers of Metabolic Health.

observational

Fatahimeiabadi Z, Apaflo JN, Bajpeyi S · American journal of physiology. Endocrinology and metabolism (2026)

Participants: N/A
Duration: 3-hour OGTT (single visit)
Intervention: Classification by glucose curve morphology (monophasic vs. biphasic) and glucose peak time (early ≤30 min vs. late >30 min) during a 3-hour oral glucose tolerance test (OGTT)
Outcome: Post-prandial glucose, 3-hour glucose AUC, insulin AUC, insulin sensitivity (clamp), FFA, energy expenditure, sleep respiratory quotient, body fat
Effect Size: N/A (p-values reported, no effect sizes)
Population: Normoglycemic adults (N=85; 40 males, 45 females; mean age 26.9±4.6 years; BMI 23.1±2.4 kg/m²)

Result:

Mechanism Graph

Early glucose peak reflects faster insulin response and better glucose disposal
Lower insulin AUC and FFA AUC indicate improved insulin sensitivity and metabolic flexibility
Higher energy expenditure and lower RQ suggest greater fat oxidation and metabolic health
Result: Early peak time identifies individuals with better metabolic health

Limitations

  • Cross-sectional design; causality cannot be inferred
  • Sample size relatively small and limited to young, normoglycemic adults; generalizability to other populations uncertain
  • No long-term follow-up to assess diabetes risk prediction

Frequently Asked Questions

What is glucose peak time and why is it important?

Glucose peak time is the time at which blood glucose reaches its maximum during an OGTT. An early peak (≤30 min) indicates faster insulin response and better glucose tolerance, while a late peak (>30 min) is associated with worse metabolic health.

How does glucose peak time compare to glucose curve shape?

This study found that glucose peak time was a stronger discriminator of metabolic health than curve shape (monophasic vs. biphasic). Early peak time was associated with multiple metabolic benefits, whereas biphasic shape only showed lower post-prandial glucose without other differences.

Can glucose peak time be used to predict diabetes risk?

The authors suggest that glucose peak time could serve as an additional biomarker from the OGTT for identifying potential diabetes risks, but further studies are needed to establish its predictive value.

What were the main differences between early and late peak groups?

Early peak group had lower post-prandial glucose, lower insulin AUC, lower FFA AUC, lower respiratory quotient, lower body fat, and higher energy expenditure, with a trend toward higher insulin sensitivity (p=0.0592).
