HD-tDCS over the dorsal medial prefrontal cortex reduces insomnia severity and improves sleep onset latency and sleep efficiency in chronic insomnia patients

A 10-day regimen of active high-definition transcranial direct current stimulation (HD-tDCS) targeting the bilateral dorsal medial prefrontal cortex significantly decreased Pittsburgh Sleep Quality Index (PSQI) scores compared to sham stimulation in chronic insomnia patients. Exploratory polysomnography findings also showed improvements in sleep onset latency and sleep efficiency after active treatment.

Result: Improved sleep initiation and maintenance, reduced insomnia severity

Limitations

  • โš Small sample size (n=55) limits generalizability

  • โš Exploratory nature of PSG findings without correction for multiple comparisons

Frequently Asked Questions

What is HD-tDCS and how does it differ from conventional tDCS?โ–ผ

HD-tDCS uses a high-definition array of small electrodes (e.g., one anode and four cathodes) to more focally target brain regions, such as the DMPFC, compared to conventional tDCS which uses larger sponge electrodes.

How long does it take for HD-tDCS to improve insomnia symptoms?โ–ผ

In this study, improvements were observed after 10 daily sessions of stimulation, with PSQI scores decreasing and sleep onset latency and efficiency improving post-treatment.

Is HD-tDCS safe for chronic insomnia patients?โ–ผ

The study reports no adverse effects, but the authors note that additional safety data are needed before it can be recommended as a routine therapy.

Can HD-tDCS replace medication or cognitive behavioral therapy for insomnia?โ–ผ

The authors suggest HD-tDCS is a possibly promising supplement to medication and CBT-I, not a replacement, and requires replication.
