HRV biofeedback may enhance HRV during stress recovery but does not improve resting HRV, emotional health, or cognition in older adults

In a randomized sham-controlled trial, 53 healthy older adults received either active HRV biofeedback or sham for 5 weeks. The active group showed a significant increase in low-frequency HRV during recovery from a stressor, but no group differences were found for resting HRV, emotional functioning, or cognitive performance. Both groups reported improved sleep quality, suggesting a possible placebo effect.

Participants: N/A
Duration: 5 weeks
Intervention: Five-week heart rate variability biofeedback intervention (active vs. sham)
Outcome: Resting HRV, HRV during stress recovery (LF, HF, RMSSD, SDNN), emotional functioning, sleep quality, cognitive performance
Effect Size: N/A
Population: 53 healthy older adults

Mechanism Graph

HRV biofeedback training involves slow-paced breathing to increase baroreflex gain and vagal tone
โ†“
Enhanced vagal modulation may improve autonomic flexibility during stress recovery
โ†“
Increased low-frequency HRV during recovery reflects improved baroreflex and sympathetic-vagal balance
โ†“
Result: Significant increase in LF during recovery, but no resting HRV changes

Limitations

  • โš Small sample size (n=53) may limit statistical power
  • โš Sham condition also improved sleep quality, suggesting possible placebo effects
  • โš No blinding of participants to group assignment was mentioned
  • โš Short intervention duration (5 weeks) may be insufficient to produce broader effects

Frequently Asked Questions

Does HRV biofeedback improve sleep in older adults?โ–ผ

Both active and sham groups reported improved sleep quality, so the improvement may not be specific to HRV biofeedback.

What is the effect of HRV biofeedback on resting heart rate variability?โ–ผ

No significant differences were found between active and sham groups in resting HRV measures (HF, RMSSD, SDNN).

Can HRV biofeedback improve cognitive performance in older adults?โ–ผ

No significant improvements in cognitive performance were observed in this study.

What does the increase in low-frequency HRV during recovery indicate?โ–ผ

It suggests potential modulation of autonomic regulation during stress recovery, possibly reflecting improved baroreflex function.
