Polysomnography in systemic mastocytosis patients shows frequent sleep-disordered breathing, REM-related changes, and variable arousal burden, but findings cannot be attributed directly to SM due to overweight/obesity predominance and lack of control group.

In a single-center observational study of nine adult patients with systemic mastocytosis (SM), polysomnography revealed that eight of nine patients met criteria for obstructive sleep apnea (OSA), with five mild and three severe cases. Median AHI was 8.2 events/hour, and all overweight/obese patients had OSA, while the only normal-weight patient did not. Brain fog-like complaints were common (six patients), but excessive daytime sleepiness was present in only two patients.
