Subjective sleepiness and objective sleep propensity do not closely align in adults with ADHD referred for sleep evaluation

In adults with ADHD referred for MSLT, subjective sleepiness (ESS scores) did not correlate with objective sleep propensity (mean MSLT sleep latency), while a significant inverse correlation was observed in the EDS-only subgroup. Median ESS scores were similar between groups (14.0 vs 13.0), but objective sleep latency was longer in the ADHD group (432.0 s vs 322.0 s, p=0.008) and MSLT positivity was less frequent (61.8% vs 87.1%, p=0.001).

Limitations

  • โš Retrospective cross-sectional design limits causal inference
  • โš Clinical referral population may not generalize to all adults with ADHD
  • โš No control for medication effects or comorbid sleep disorders

Frequently Asked Questions

Does the Epworth Sleepiness Scale accurately reflect objective sleepiness in adults with ADHD?โ–ผ

No, the study found no significant correlation between ESS scores and mean MSLT sleep latency in the ADHD group, suggesting subjective complaints may not match objective measures.

How do objective sleep measures differ between ADHD and non-ADHD patients with excessive daytime sleepiness?โ–ผ

Adults with ADHD had significantly longer mean sleep latency (432 s vs 322 s) and lower rates of MSLT positivity (61.8% vs 87.1%) compared to the EDS-only group.

What is the clinical implication of this study for ADHD patients reporting daytime sleepiness?โ–ผ

The findings support the need for integrated psychiatric and sleep-medicine assessment, as subjective sleepiness may not align with objective sleep propensity in this population.

What was the sample size and study design?โ–ผ

This was a retrospective cross-sectional study of 130 adults (68 ADHD, 62 EDS-only) who underwent MSLT between January and December 2021.
